Tutors: You must fill this form out completely before submission. You cannot save your work and come back to it later because the form requires both your electronic signature and the student's. If you do not have time to use this form, you must use the paper form instead. Online session records are due immediately following the session, paper session records must be submitted within 48 hours following the session. DO NOT use the Enter key - click the SUBMIT button.

result in a violation of the Student Code of Conduct (furnishing false information
to any member of the university community) and will be forwarded to the Office
